Going home after hospital stay

If you’re a Northern Territory resident aged over 65 (non-Indigenous) or over 50 (Indigenous) and are returning home after a hospital stay, you may be eligible for support services. 

You will be assessed by a Discharge Care Coordinator (DCC) while you're still in hospital to find out what services you may need.

There are various services to suit your level of ability.

Community Home Support Package

This service helps you with assistance at home including  personal cares, meals,  cleaning, shopping and transport.

The Discharge Care Coordinator can refer you to this service through the My Aged Care portal.

Meals on wheels

The Discharge Care Coordinator can refer you to this service. The meals on wheels service includes a fee.

Regis Therapy Centre

The Discharge Care Coordinator can refer you to this service for group or individual therapy. This therapy service includes a fee.

Transitional Care Package

This is a 12 week program and includes low intensity therapy and personal and/or nursing care

To qualify for this service, you must be assessed by the Aged Care Assessment team before leaving hospital.

The Discharge Care Coordinator can help you to contact the Aged Care Assessment team (ACAT) through the My Aged Care portal.

Allied health will also assess your therapy goals and complete a report for ACAT outlining your goals. 

You can't self refer for this service.

Home care packages

Home care packages provide a more flexible and tailored package of care to enable an older person to remain at home.

To be approved you must be assessed by the Aged Care Assessment team (ACAT) before leaving hospital.

The Discharge Care Coordinator can help you to contact ACAT through the My Aged Care portal.

ACAT will determine what level of packaged care you need.
