Aged care services

There are services, support and care available for older people in the NT.

You can access these services if you are:

  • Aboriginal and aged 50 or older
  • non-Aboriginal and aged 65 or older

Australian Government-funded supports

The Australian Government funds supports and services for older people. NT Health delivers four of these services:

  • Commonwealth Home Support Program (CHSP) - helps you get entry-level services so you can keep living safely and independently at home.
  • Transition Care Program (TCP) – helps you recover after a stay in hospital.
  • Multi-Purpose Service (MPS) program - helps older people in rural and remote areas to get health and aged care services close to home.
  • Aged Care Assessment Service (ACAS) - helps you find out what aged care services you need and how to get them.

The CHSP helps you get entry-level services so you can keep living safely and independently at home.

Fees

  • Individual social support - $5 per hour
  • Group social support - $5 per session

Equipment and home modification

The CHSP also loans basic equipment for:

  • showering
  • going to the toilet
  • transfer and mobility
  • home modifications, such as grab rails, handrails and ramps.

An occupational therapist will work with you to determine what equipment or home modification you may need.

The TCP costs $94.49 a week. If you cannot pay this much, we can work out a cost that you can afford.

The MPS program helps older people in rural and remote areas to get health and aged care services close to home.

Fees and payment

The MPS program has variable fees that depend on the type of service provided and the provider.

There is no formal income or assets test. However, NT Health will assess your ability to pay in a fair and reasonable manner.

If you receive compensation that covers care costs, NT Health may charge the full cost of the service.

You may be asked to pay for services in advance. However, payments will not be more one month in advance.

You have the right to request a review of your contribution amount at any time. This may be when your financial circumstances change or you experience hardship. Requests for review will be considered promptly and respectfully.

Older Persons Wellbeing Program

This service provides support to older people experiencing mental health symptoms, with age related physical conditions.

The program works with you to:

  • modify symptoms affecting your wellness and quality of life
  • link you with services of your choice
  • provide information to you, family and providers to understand your needs
  • find strategies to help manage your symptoms.

Memory Service

This service provides a diagnostic pathway and support to eligible people with cognitive impairment who live in the Top End.

The team includes:

  • the community geriatrician
  • an Australian Government's Home and Support Program dementia nurse
  • an occupational therapist.
