This page has information for NTCAT applicants about presenting evidence and witness statements at a hearing.
Apply to present evidence at a hearing
If you would like to require a person to give evidence at your hearing, you may apply to NTCAT for an evidence summons. To apply follow the below steps.
Step 1. Fill in:
- an application for evidence summons form and
- an evidence summons form.
You can get both forms from the forms section of the NTCAT website.
Step 2. Submit both forms in any of the following ways:

In person
Submit your form at your nearest NTCAT office. If you live in a remote area you can submit your application and fee at your local NT court facility.

Witness declaration statements
To submit a witness statement as part of your case, follow these steps:
Step 1. Get the witness to fill in an unattested declaration.
You can get the declaration from the forms section of the NTCAT website.
Step 2. Submit the form by email to AGD.ntcat@nt.gov.au or in person.
