Collection notices

The Department of Education and Training, including schools, collects personal information about students and their families to provide education and support services.

This information is collected when parents complete a form, on paper or online.

Depending on the form, we may collect:

  • names
  • contact details
  • addresses
  • medical information
  • emergency contacts
  • details about a student’s learning needs and achievements.

This information is collected in line with the Information Act 2002.

What it is

A collection notice is a short statement on a form that explains:

  • why the school or department is asking for information
  • how the information will be used
  • who the information may be shared with.

It helps you understand your rights and obligations before providing personal information.

You may see a collection notice on:

  • enrolment forms
  • student advocacy services forms
  • excursion and camp permission forms
  • career and vocational education and training (VET) program forms
  • work placement agreements
  • online learning tools and digital applications.

Why we collect it

Information is collected to:

  • enrol students and keep records
  • support learning, wellbeing and safety
  • provide advocacy and counselling
  • organise VET, work experience and school programs
  • give access to digital learning tools and school communications
  • meet legal and reporting obligations.

How we use and share it

Information is used to help your child participate in school and get the right education and supports.

It may also be used for planning and improving the quality of education programs.

We may share information with:

  • other schools or training providers, for student transfers or VET and work experience programs
  • government agencies where required by law
  • approved service providers that support education or wellbeing programs.

We keep your information safe and only share what is needed.

How it is protected

Information is:

  • collected by lawful and fair means
  • stored securely in school or department systems and areas
  • only used for the purposes stated in the collection notice.

Unauthorised disclosure is prohibited and may result in disciplinary or legal action.

Your rights

When you give your information make sure you know:

  • the details of the department or school that is collecting the information
  • your right to access and request correction of your information
  • the purpose of collection
  • when your information might be shared and with who
  • any law that requires the information to be collected
  • any consequences of not providing required information, for example your child may not be able to:
    • enrol
    • go on/attend excursions
    • participate in VET or work placements
    • access digital learning tools.
