Apply for an evidence of age card
The Northern Territory (NT) evidence of age card is a form of photo identification (ID).
It expires 5 years after issue.
Who can apply
You can apply for this card if you are an NT resident aged 18 and over.
How to apply
To apply, follow the steps below.
Step 1. Get supporting documents for evidence of your identity and residency.
Step 2. Fill in the application for evidence of age card form PDF (244.0 KB).
Step 3. Submit the form with supporting documents in person at an MVR office.
You can book an appointment at MVR Darwin to apply for an evidence of age card. Book online on Territory Services.
Step 4. Pay the fee - a concession may apply.
If you're an NT Concession Scheme cardholder, you can get a free evidence of age card.
You must show your current concession card.
Renew your card
To renew, follow the steps below.
If your photo was taken more than 5 years ago
Go to an MVR office to:
- get your photo taken
- pay the fee - a concession may apply.
You will need to provide evidence of your identity and residency.
If your photo was taken within the past 5 years
You can pay the fee:
- by phone on 1300 654 628
- in person at an MVR office.
If you're an NT Concession Scheme cardholder, you can renew your card for free.
You must show your current concession card.
Replace a lost, stolen or damaged card
To replace your card, you will need to pay the fee.
If your address has changed, you must inform MVR before requesting a replacement.

Online
You can apply online if you have more than 6 months before your card is due to expire.
Other ways to apply
You can request a replacement card:
- by calling 1300 654 628
- at your local MVR office.
After you apply
Your card will be sent to your recorded postal address.
When you receive your card, you must check if all the details are correct.
If there are any errors, you must contact MVR as soon as possible.
If you change your name or want a new photo, you must apply for a new card at an MVR office.
