Renal social work

A renal social worker offers emotional support and connects you with community resources. They also support your family in managing kidney disease challenges.

The team works closely with the renal Aboriginal Liaison Officers.

Access the service

The renal social workers provide care to patients in the follow regions:

• Top End
• Big Rivers
• East Arnhem.

Outpatient referrals can only be made by a health professional.

Inpatient referrals can be made by hospital staff.

Telehealth consults are also available.

Eligibility

You can be referred to the service if you:

• have chronic kidney disease stage 3B onwards
• are on any form of dialysis.
