Hospital based social workers

Social workers can provide help when you, a family member or carer are dealing with complex health or social needs.

Hospital based social workers can provide support to patients and their families.

Social workers are a free service in NT Health hospitals, referrals are prioritised based on severity of need.

Social work services

Social worker provide:

  • crisis intervention
  • bereavement support
  • illness adjustment education and counselling
  • complex discharge planning
  • income and housing support
  • end of life care
  • case management
  • advocacy

If you're being treated in an NT hospital, you can ask to speak with a social worker at the hospital you are being treated at.

You can ask to see a social worker while you are being treated at RDPH.

Families can also access the service.

Social workers are located at RDH at the Allied Health office on the 1st floor.

You or a hospital staff can contact social workers by calling the hospital on 08 8922 8888 during business hours.

NT Health professionals can also email the service  alliedhealthadmin.doh@nt.gov.au.

There are limited outpatient services for providing crisis support. Social workers for outpatients is a referral service for NT Health services, such as from the pregnancy options clinic.

You can ask to see a social worker while you are being treated at ASH or as an outpatients including home visits to renal and palliative care units.

Families can also access the service.

Social workers are located on the ground floor of ASH.

Other staff, agencies, patients and their families can all place a referral.

Social workers work alongside Aboriginal Liaison Officers.

Katherine Hospital  social workers are specialised in domestic and family violence intervention, complex child protection, trauma and carer support.

You can ask to see a social worker while you are being treated at GDH or as an outpatient.

Families can also access the service.

There is a weekend on-call service.

You can ask to see a social worker while you are being treated at Tenant Creek Hospital or as an outpatient.
