Report a petroleum operations incident
You must report an incident that causes, or is likely to cause, any of the following:
- the death of a person
- serious injury to a person
- significant damage to a pipeline.
You must report an incident which the permit holder considers needs immediate investigation.
You must notify the department or an inspector as soon as possible after the incident - either verbally or in writing - giving as much detail as possible.
This must be followed by a full written report as soon as practicable after the incident.
